We are dedicated to making it easy to be a patient at Carolina Bone & Joint.  Please note these steps for paving the way for a smoother visit, payment, and follow-up process: (1) print, complete and bring the forms with you to your appointment and (2) review our before and after visit information below.

Before your visit

We accept most insurance plans and worker’s compensation cases. Please contact your insurance company to verify our participation with your plan, and bring your insurance card to each office visit.  If you have questions about insurance please call 704-541-3055, and select the billing option.  If you do not have insurance, please come prepared to pay in full at the time of service.

After your visit

    • If you submit disability forms, allow two weeks for processing.  A processing fee and patient questionnaire are required prior to submission of each form.
    • To obtain medical records, call 704-541-3055 and select the medical records option.  You will be asked to sign a HIPAA release.
    • Call your pharmacy to request prescription refills, allowing ample time for processing them.
